Dynamical Optical Breast Imaging (DOBI): An Innovative Method for Detecting and Characterizing Breast Vessels in Women Presenting BI-RADS 4-5 Imaging Findings

PURPOSE

To evaluate a new modality for imaging breast vessels through detection of methemoglobine by infrared photoluminescence diodes in patients scheduled for core or surgical biopsy.

METHOD AND MATERIALS

72 consecutive patients (age 41-72) presenting BI-RADS 4 (n=40) or 5 (n=32) lesions depicted by routine mammography and/or ultrasonography were studied. DOBI scans were performed immediately before the core biopsy or the preoperative wire-hook location. Breast was positioned in craniocaudal view and soft pressure was applied. 45 light acquisitions in an entirely dark environment lasted 70 seconds. The evaluated parameters included early "blush", corresponding to rapid blood inflow-outflow in the region of interest, the total number of abnormal pixels calculated and the type of dynamic time-dependent curves. Correlation to pathology was obtained in all cases.

RESULTS

41 optical acquisition scans were considered as positive in 23 BI-RADS 4 and 18 BI-RADS 5 cases. All BI-RADS 5 cases proved to be infiltrating ductal carcinomas, whereas 12 BI-RADS 4 cases corresponded to malignant lesions. The remaining 11 positive BI-RADS 4 cases were due to fibrocystic disease, ductal or lobular hyperplasia with or without atypia and radial scars. The "blush" was always larger than the real tumor size. 31 scans were negative in 17 BI-RADS 4 and 14 BI-RADS 5 cases. BI-RADS 5 cases corresponded to 6 purely intraductal and 7 small size (<5mm) infiltrating tumors. The last BI-RADS 5 was a false positive for mammography as well as DOBI corresponding to sclerosing adenosis on specimen. Histology in BI-RADS 4-DOBI negative patients revealed 11 benign and 6 malignant lesions, the latter corresponding to small DCIS.

CONCLUSION

Infrared optical breast imaging is an innovative, well-tolerated, non-ionizing method that could provide useful information regarding malignant and benign breast lesions vascularization.
