Adult Cystic Fibrosis: Intra and Interobserver Variability and Correlation with Clinical Parameters of Two Different Chest Radiograph Scoring Systems

PURPOSE

Most of chest radiograph scoring systems for cystic fibrosis were developed for children but they are used in adults.This study was designed to compare intra and inter observer variability of two different chest radiograph scoring systems (Brasfield and Chrispin-Norman) and to correlate them with clinical parameters.

METHOD AND MATERIALS

The patients chosen for the study were all older than 18 years old.The study took part at the cystic fibrosis unit of the Hospital de la Princesa (Madrid). Twenty-four chest radiographs were scored according to Brasfield and Chrispin-Norman scoring systems by two independent observers. Data on clinical parameters such as pulmonary function tests (PFT), nutritional status, gender, age, etc, obtained simultaneously with the chest radiograph, were reviewed. Intra and interobserver agreement were calculated by Pearson correlation coefficient and intraclass correlation coefficient (ICC). The scores were correlated with pulmonary function tests and other clinical data.

RESULTS

Intra and interobserver variability was low (r ≥ 0.9, ICC ≥0.85 all p<0.001), and scores had good limits of agreement (Bland and Altman). Correlation of radiograph scores with clinical parameters were good for most pulmonary function test: FEV1 (r = 0,64 y 0,55); FEV1% (r = 0,75 y 0,72); y FVC % (r = 0,63 y 0,056). There were not good relations with gender, age nor body mass index. Hospital admission in the previous year was associated with increased radiological alterations.

CONCLUSION

Both radiographic scoring systems (Brasfield and Chrispin-Norman) showed a low intra and inter observer variability and correlated well with pulmonary function tests, especially FEV1% predicted .
