To compare the dual-phase helical CT, Mn-DPDP enhanced MRI, and combination of Mn-DPDP MRI and CT in the detection of hepatic metastases from the colorectal cancers.
Thirty-three colorectal cancer patients with 43 metastatic nodules underwent dual-phase (arterial and portal) helical CT scan and Mn-DPDP MRI before the surgery. The indication of MRI was presence or suspected of having metastatic lesions at CT scans and/or increased serum carcinoembryonic antigen (CEA) levels (10 ng/mL or more). The diagnosis was established by the combination of findings at surgery, intraoperative ultrasonography, and histopathologic examination. T2-weighted MR images were obtained with HASTE sequence. T1-weighted images were acquired before and 2 hours after intravenous administration Mn-DPDP using FLASH sequence. Three radiologists interpreted CT, MRI, and combination of CT-MRI at discrete sessions and evaluated each lesion with agreement by consensus for lesion location, lesion size, and intrinsic characteristics. The lesions were divided into three groups (p < 1 cm, 1≤q<2 cm, r ≥2 cm) according to their diameter.
CT, MRI, and combined CT-MRI diagnosed 79, 51, and 37 nodules as metastases, respectively. The sensitivity was 65% (p=35%, q=69%, r=100%) for dual-phase CT, 65% (p=47%, q=69%, r=100%) for MRI, 77% (p=59%, q=77%, r=100 %) for a combination of CT-MRI. Positive predictive value was 35.4% (28/79), 54% (28/51), and 89.1% (33/37) for each modality, respectively.
CT scan or MRI alone could show many false positive lesions without increase of detection rate of metastatic nodules. Combination of CT and MRI is more sensitive and accurate than CT or MRI alone in the detection of hepatic metastases from the colorectal carcinoma.
