ABSTRACT

Hepatic perfusion abnormalities are frequently observed on dynamic contrast-enhanced computed tomography (CT) and magnetic resonance (MR) imaging. These perfusion abnormalities, which manifest as a transient hepatic intensity difference (THID) on MR and a transient hepatic attenuation difference (THAD) on CT, are significant because they may mimic other pathology and cause diagnostic confusion. Furthermore, they often serve as key diagnostic clues to the presence of important hepatic and extrahepatic disorders. This educational exhibit discusses: (1) the physiological basis of THID and THAD and their imaging appearances; (2) differentiation of THID and THAD from other hepatic lesions; (3) recognition of hepatic and extrahepatic disorders associated with THID and THAD; and (4) diagnostic pitfalls. Fundamental concepts are emphasized with schematic diagrams and correlative CT/MR imaging. Algorithms for interpretation and workup are proposed based on review of the latest literature.
