Organized Hematoma of the Maxillary Sinus: CT Imaging Features

PURPOSE

To describe CT imaging features of organized hematoma of the maxillary sinus.

METHOD AND MATERIALS

CT images of total 13 patients(9 males and 4 females, mean age: 51.5 years) with pathologically or surgically confirmed organized hematoma were reviewed. Eight patients obtained both pre- and post-contrast enhanced CT scan, three patients obtained pre-contrast enhanced CT only, and one patient obtained post-contrast enhanced CT only. All patients presented with a history of hemoptysis. CT findings were evaluated retrospectively on the base of presence of the antral expansion or bony erosion, and CT density. Histologically, the resected sample consisted of amorphous fibrin masses mixed with hematoma and neovascularization.

RESULTS

The maxillary sinus showed high density on the pre-contrast enhanced CT in 13 cases(n=13) and scattered enhancement with amorphous pattern in 10 cases (n=10). Localized or diffuse expansion of the maxillary sinus was noted in 11 cases(n=13). Bony erosion of the antral wall was noted in 10(n=13).

CONCLUSIONS

Oganized hematoma should be included in the differential diagnosis when patients have frequent epistaxis with the CT findings showing expansion of the maxillary sinus, high density on pre-contrast enhanced CT and amorphous enhancement on post-contrast enhanced CT.
