Comparison of Arterial Spin Labeling and Dynamic Susceptibility-weighted Contrast-enhanced MR Imaging in Patients with Acute Cerebral Ischemia at 3.0T

PURPOSE

To implement an arterial spin labeling(ASL) technique that is feasible in routine examinations and to test the method and compare it with dynamic susceptibility-weighted contrast-enhanced MR imaging for evaluation of acute cerebral ischemia at 3.0T.

METHOD AND MATERIALS

Both pulsed arterial spin labeling(PASL) technique and dynamic susceptibility-weighted contrast-enhanced MR imaging were performed on 26 patients with the clinical diagnosis of acute stroke(14 men,12 women,age range 33 to 87 years,mean age 62 years). For qualitative analysis,perfusion studies of both techniques were grouped into categories(hyperperfusion,normal perfusion,delayed perfusion,or absent perfusion)and compared.Quantitative analysis was performed using signal intensity measurements in a region of interest that was defined by diffusion-weighted imaging abnormalities and a mirror region in the contralateral unaffected hemisphere respectively.Signal intensity ratios(infracted region versus the unaffected contralateral region) were calculated and compared using a paired t test.

RESULTS

Qualitative analysis demonstrated agreement between the two techniques in 21 of 26 studies(hyperfusion,n=2 patients;normal perfusion,n=3;delayed perfusion,n=5;and absent perfusion,n=11).Quantitive analysis revealed there was no significant difference between two techniques(p>0.05).

CONCLUSIONS

The noninvasive PASL technique was as sensitive as dynamic susceptibility-weighted contrast-enhanced MR imaging in detecing perfusion abnormalities in the areas of decreased diffusion.and may provide a valuable alternative in the diagnosis of acute cerebral ischemic stroke patients.
