Perfusion Study of Hepatic Tumors: Use of Multi-Detector Row Helical CT and Liver Perfusion Software

PURPOSE

To compare and assess CT perfusion parameters and images of hepatic cell carcinoma(HCC) and hypervascular benign hepatic tumor by using multi-detector row helical CT and liver perfusion software.

METHOD AND MATERIALS

Hepatic perfusion scans were performed in 29cases with HCC and 23cases with hypervascular benign hepatic tumor with pathologically or clinically proven. The studies were performed with cine mode on a MSCT and liver perfusion software.

RESULTS

The blood flow(BF:ml/min/100g),blood volume(BV:ml/100g),mean transit time(MTT:s),permeability surface area product(PS:ml/min/100g),hepatic arterial fraction(HAF),time of arrival and base image(BI) of HCC lesions were 302.62±201.66,33.95±23.01,9.42±3.93, 27.61±15.12,0.60±0.22,1.59±1.18s and 48.14±6.94HU,respectively. The mean BF,BV,MTT,PS,HAF,time of arrival and BI of normal liver parenchyma around HCC lesions were 155.81±95.41,16.48±8.12,12.89±6.05, 32.29±14.02,0.22±0.14,1.67±1.32s and 62.69±6.89HU,respectively. The mean BF,BV,MTT,PS,HAF,time of arrival and BI of hypervascular benign hepatic tumor lesions were 247.38±219.97,32.71±35.07,12.38±2.98s,26.81±13.15,0.51±0.22,1.60±0.96s and 49.98±3.91HU,respectively. The mean BF,BV,MTT,PS,HAF,time of arrival and BI of normal liver parenchyma around hypervascular benign hepatic tumor lesions were 113.75±37.75,18.02±5.46,15.01±2.78, 3.87±12.16,0.13±0.06,1.89±1.20s and 64.01±8.13HU,respectively. The mean BF,BV,MTT,HAFand BI confirm statistically significant differences between HCC lesions and normal liver parenchyma around them(P=0.000,0.000,0.006,0.000 and 0.000).The mean PS,HAF and BI confirm statistically significant differences between hypervascular benign hepatic tumor lesions and normal liver parenchyma around them (P=0.02,0.000 and 0.001).The mean MTT in HCC lesions was much shorter than in hypervascular benign hepatic tumor lesions (P=0.018).The mean HAF in normal liver parenchyma around HCC lesions and hypervascular benign hepatic tumor lesions were statistically significant (P=0.004),and the former was much larger.

CONCLUSION

Hepatic perfusion parameters can confirm significant difference between hepatic tumor lesions and normal parenchyma around them,and between malignant and benign hepatic tumors.
