Proton MR Spectroscopy of the Subependymal Tubers Near to the Foramen of Monro in Children with Tuberous Sclerosis Complex without Giant Cell Astrocytoma

Abstract: HTML Purpose: one of the most harmful complication in patients with tuberous sclerosis complex (TSC) is the malignant transformation of subependymal nodules, which is specially frequent around the foramen of Monro. The proton MR spectroscopy is a highly sensitive technique in the detection of neoplasia, and it can be potentially useful in the early screening of the malignant transformation observed in TSC. The authors aim to present the MR proton spectra of the subependymal nodules near to the foramen of Monro in children with TSC without giant cell astrocytoma (GCA). Methods and Materials: follow-up CT scans and MRI were studied to exclude the presence of a giant cell astrocytoma (GCA). Proton spectroscopy was performed with a multi-voxel spectroscopy technique (PRESS, TE 144 ms) over the foramen of Monro region bilaterally. The results were expressed in terms of the ratio of the area under the three prominent resonances of cholina (Cho), creatine (Cr) and N-acetyl groups (NA). Results: twelve children with TSC were studied. There were nine female and three male patients, with age averaging of 10 years. The NA/Cho ratio was 1.35 (+-0.19) and the Cho/Cr ratio was 0.86 (+-0.12). Conclusion: our study show the NA/Cr and Cho/Cr ratios of the subependymal nodules around the foramen of Monro in patients with TSC without GCA. These results can be compared with future studies of patients with TSC and GCA to demonstrated the value of the proton MR spectroscopy in the follow-up of patients with TSC, with the objective of early detection of GCA.       Questions about this event email: gasparetto@hotmail.com
