Purpose: To evaluate the role of magnetic resonance (MR) imaging in
evaluating psoriatic arthropathy
Methods and Materials: Sixteen patients (M:F=11:5, mean age 45.9 yrs) with
active psoriatic arthropathy (wrist 6, hand 5, knee 3, foot 2) who had positive
findings on plain radiographs were analyzed. MR images were obtained on at
least two orthogonal planes and each examination included at least one FSE T2,
gadolinium-enhanced T1 with fat saturation, and 3D gradient sequences. Using
our home-developed scoring system, bone marrow edema, bone erosion, synovial
proliferation, joint effusion, bone proliferation/ enthesopathy, and
para-articular soft tissue change were evaluated and scored for the T2 signal
intensity, the degree of enhancement, and extent. A four-point scale of 0 to 3
was. American College of Rheumatology (ACR)s clinical scoring system (a 4 point
scale) was used for pain and swelling assessment. The score sum of each MR
finding was compared with the clinical score. Four musculoskeletal radiologists
evaluated the images by consensus and 1-tail paired t-test was used for
statistical analysis.
Results: Synovial proliferation and bone erosion were observed in all 16 cases.
Bone marrow edema was found in 83%, para-articular soft tissue change and bone
proliferation were found in 63% and joint effusion was present in 56% of cases.
BM edema, bone erosion, and synovial proliferation showed a significant
correlation with patients pain score, swelling score, and combined score. Joint
effusion and bone proliferation did not have a significant correlation with
clinical disease severity.
Conclusion: MR can be a useful imaging modality for psoriatic arthritis and may
be used for a semi-quantitative assessment using our scoring system.
