Purpose: To describe the MR features to differentiate lumbar discal cyst from other spinal epidural cysts.
Methods and Materials: MR images of 9 patients (7 male and 2 female, mean age: 32.2 years) with surgically confirmed discal cyst were reviewed. All patients presented with a history of low back pain and radiculopathy. All patients underwent partial hemilaminectomy and resection of the cysts. MR findings of cysts and their relations to degenerative disc disease were evaluated.
Results: Discal cysts were located at L4-5 in 4 (44.4%), L2-3 in 2, and others. All cysts were seated in the antero-lateral epidural space of the spinal canal without extension to the neural foramen or midline crossing. All cysts had hypointense rims on T2-weighted image and five revealed dense rim enhancement after contrast injection. Bulging or herniation of the adjacent intervertebral disc was associated in all. On resection of the cyst, the cyst was tightly adherent to the annulus fibrosus without connection to the facet joint. Histologically, the resected sample consisted of thick fibrous capsule with myxoid degeneration.
Conclusion: MR imaging features of lumbar discal cyst is different from synovial cyst of the facet joint. This uncommon epidural cyst is thought to be closely related to degenerative disc disease.
