Imaging Approach to Resectability Evaluation of Suprapancreatic and Hilar Cholangiocarcinoma

Abstract: HTML Patients with cholangiocarcinoma have a poor prognosis at the time of diagnosis. Surgery offers the best chance for cure particularly with hepatic lobectomy, extended lobectomy, or liver transplantation with margin-negative resection of the tumors. However, only 20-40% of patients were candidate for curative resection partly because of the local invasiveness of the tumor. Preoperative imaging studies including US with color Doppler, multiphasic helical CT, and MRI and MRCP are commonly used for pretreatment evaluation of these patients to identify those who are surgical candidates and for planning surgery. The purpose of this exhibit is to use state-of-the-art imaging techniques to show: 1) hepatic arterial and portal venous anatomy in the hepatoduodenal ligament and the hilar fissure, 2) the relationship between these vessels and the tumor, 3) the criteria of vascular involvement and 4) the criteria of resectability and non-resectability.     1.
