TEACHING POINTSDifferentiating an atypical leiomyoma from a leiomyosarcoma is challenging. Leiomyosarcomas are aggressive tumours, hence early detection and treatment is key for patient survival. Classical concerning features such as rapid growth of a lesion in a post-menopausal patient can be observed in both atypical leiomyomas and leiomyosarcomas and may not be available at the time of image interpretation. Using a combination of qualitative MRI features in the acronym BET¹T²ER Check! may help identify concerning lesions aiding optimal appropriate management.
TABLE OF CONTENTS/OUTLINEThe exhibit will provide a comprehensive pictorial review of MRI findings including: Classical features of leiomyomas, Atypical imaging appearances of benign leiomyomas including cellular and degeneration subtypes; myxoid, cystic and red degeneration, Classical appearances of leiomyosarcomas, Pearls and pitfalls of differentiating atypical leiomyomas from leiomyosarcomas, including using the acronym; B: Borders - Nodular or smooth, E: Enhancement - Central unenhanced areas, T1: T1 intralesional haemorrhage, T2: T2 dark areas, E: Endometrial interface, R: Restricted diffusion, Combining key imaging features to improve diagnostic accuracy, Pearls and pitfalls in distinguishing common mimics from leiomyomas and leiomyosarcomas will be highlighted.
