Comparison of One-stage and Two-stage Embolization of the Uterine Artery in Treatment of Symptomatic Uterine Fibroids

Abstract: HTML Purpose: To evaluate and compare the radiation dose, efficacy and safety of the uterine arteries (UAE) in the management of symptomatic uterine fibroids with different embolization techniques. Methods and Materials: We analyzed 60 women (mean age, 39.2 years) undergoing UAE for the treatment of symptomatic uterine fibroids with consecutive unilateral embolization using a biphasic protocol (n=35) or bilateral embolization in a single treatment protocol (n=25) with polyvinyl alcohol particles (n=54) or microspheres (n=6) as embolic agent. Radiation dose was recorded in all patients. Devascularization and size of the fibroids were achieved with contrast enhanced MRI at 3 months and 12 months after therapy. Side effects and changes in clinical symptoms after therapy were evaluated. Results: All procedures were technically successful. The mean fluoroscopic time during single embolization was 12.42 minutes and during biphasic embolization 11.32 minutes. The clinical outcome after UAE with both embolization techniques were comparable. After a mean follow-up duration of 14 months (range, 6 to 18 months), all women reported comparable improvement in their symptoms and resumed a reduction of the menorhagia after the procedure with both procedures. The biphasic protocol was associated with a minor degree of pain after UAE. MRI studies showed a comparable reduction of the fibroid diameter for both embolization procedures with significant decrease in 49 (82%) of the patients with a reduction of the tumor volume of 40-70% after 12 months. Conclusion: A bilateral femoral access during a biphasic UAE treatment results in a reduction of the radiation dose during the embolization with comparable clinical outcome and minor pain symptomatic after the embolization.
