Purpose: To evaluate the effectiveness of intra-arterial infusion of dilute
lidocaine for providing pain relief during UFE.
Methods and Materials: 87 patients underwent UFE. 58 were given a diluted
Lidocaine solution with 40mg in 6.0 ml and 29 patients(control group) received
a saline solution 5.0 ml through the catheter after polyvinyl alcohol (PVA)
embolization and before injection of 1 to 3 gelfoam sticks. All of the patients
were hospitalized for at least 3 days after the procedure. A questionares was
collected by the attending residents to record the vasospasm, patient pain
numeric rating scale score from the beginning of the procedure through three
days after procedure. The acute post-operative pain and other symptoms were
recorded and compared. The patients were followed-up by a contrast CT scan
after 6 months.
Results: Compared with control group, local intra-arterial anesthesia protocol
with dilute lidocaine for UFE yielded good results for pain relief during and
after the procedure. No interfering side effect was observed during the
procedure. The patients in the lidocaine group experienced slight pain after
UFE and no morphine was required. In the pain scale statistical results, there
is a siginificant difference between two groups (p<0.01). Menorrhagia and
bulk symptoms improved equally in both groups and the fibroid and uterine size
were lessened in a 6 month follow-up in all of the patients.
Conclusion: Intra-arterial use of diluted Lidocaine solution in UFE, can
dramatically relieve the pain during and after the procedure. This improved UFE
protocol is a simple approach to prevent the post-operative pain with good
results.
