Purpose: To present initial results of uterine arteries embolization as a treatment of symptomatic adenomyosis.
Methods and Materials: Prospective series of 15 patients (32-57 year, average 45.5) referred for pelvic pain (n=3), menometrorrhagia (n=4), or both (n=8) ; in whom MRI showed adenomyosis either diffuse (n=9), or focal (n=6) with no fibroid (n=10) or non dominant fibroids (n=5). Bilateral selective uterine artery embolization with calibrated microparticles (700-900 μm, Embospheres ®, BioSphere Medical, Inc.Rockland, MA, USA). Comparative analysis : 1) quantitative: uterine volume, junctional zone thickness at repeat MRI, 2) qualitative : intensity of clinical symptoms, impact on quality of life ; before, at 3 month and 6 month after embolization.
Results: Selective uterine artery catheterization was possible in 97% of cases (29/30 arteries). Injected particles volume :0.5-4 ml (average 1.20ml by side, 2.75ml by patient). At 6 month, mean uterine volume reduction : -32% (range 0-60%), mean junctional zone reduction : -2mm (range 0-8mm). Improvement in clinical symptoms : 12/15 patients (80%), disappearance of clinical symptoms n=9/15, marked improvement n=3/15. No improvement or aggravation n=3/15. No serious complications occurred.
Conclusion: Uterine arteries embolization allows a significant improvement of clinical symptoms at 6 months in 80% of patients. It could be a new therapeutic modality for symptomatic adenomyosis. Further studies are warranted.
