The Value of US-monitoring of Acute Prostatitis Associated with Vesiculitis

Abstract: HTML Purpose: To evaluate the possibilities of Pulse Doppler, Power Doppler and 3D PD TRUS in monitoring response to treatment in patients with acute prostatitis associated with vesiculitis. Methods and Materials: 32 patients (age range, 27-62 years) with acute prostatitis and vesiculitis underwent TRUS before, during and after treatment on EUB-6000 (Hitachi) and HDI-5000 (ATL-Philips). System settings were constant in all patients in B-mode and Power Doppler. In gray-scale seminal vesicles' characteristics (diameter, structure, symmetry) were evaluated. Peak systolic velocity (PSV) and resistive indices (RI) in the seminal vesicles' arteries were detected with Pulse Doppler. Vascularity degree (VD) of the seminal vesicles was evaluated using 2D and 3D PD TRUS (grade 0-2). US changes were evaluated before and after treatment. TRUS-results were compared with clinical and laboratory data in each patient throughout the follow-up study. Results: Before treatment seminal vesicles were enlarged (>1,6sm - 84%) with asymmetry in 75%(24) of cases and cystic structure - in all cases. VD grade 2 was showed in 75 % of cases. Increased PSV (range 6,0-14,3 sm/s) and RI (range 0,56-0,81) were detected before treatment in 87% of patients. Gray-scale TRUS showed positive changes in prostate echogenicity and volume, vesicle's diameter and structure in 71% (23) during and in 92% (29) after treatment. PSV (range 2,7-4,8 sm/s) and RI (range 0,28-0,45) decreased gradually during the treatment. TRUS grey scale and VD grade 0 was detected in 91% of cases after treatment. Power Doppler was superior to grey scale and Pulse Doppler in monitoring response to treatment in acute vesiculitis. Conclusion: Power Doppler and 3D PD TRUS are useful for assessment of the response to treatment in patients with acute prostatitis associated with vesiculitis.       Questions about this event email: mkisliakova@yandex.ru
